The action against the illegal codeine cough syrup network in Uttar Pradesh has picked up strong momentum. Authorities have so far registered 128 FIRs, cancelled 280 drug licences, seized lakhs of bottles of cough syrup, and arrested 32 main accused. These figures clearly show the seriousness and scale of the crackdown.

As the Yogi Adityanath government moves firmly to dismantle this organised syndicate, another effort is unfolding in parallel. Misinformation and half-truths are being spread to confuse the public. Incidents involving cough syrups from other states, which have no connection to Uttar Pradesh, are being selectively highlighted and wrongly linked to this case. There is a real chance that many people, including you, may have come across such misleading claims on social media or messaging platforms.

To separate facts from rumours, it is important to understand what the Uttar Pradesh cough syrup case is actually about and what it is not.

Understanding the Uttar Pradesh cough syrup case

As explained earlier in detail, this case centres on an organised criminal network involved in the illegal stocking, sale, and trafficking of prescription-only cough syrups that contain codeine, which is an opioid drug.

Codeine is a substance derived from opium. It is commonly used in cough syrups for medical purposes, but if taken in large quantities, it can produce mind-altering effects. Prolonged or excessive use of codeine can lead to addiction, similar to drugs like heroin or opium. Over the past few years, its misuse as a soft drug has increased steadily.

The cough syrups in question cannot be sold without a valid doctor’s prescription. However, investigations found that these medicines were being sold illegally by using fake paperwork and forged records. These documents were created to justify keeping unusually large stocks of the syrup. Once stocked, the bottles were moved through shell companies and then smuggled across state borders.

Further investigation revealed that the network was not limited to Uttar Pradesh alone. Its links extended to Kashmir and West Bengal, and even crossed the international border into Bangladesh. Acting on specific intelligence inputs, the Uttar Pradesh Special Task Force carried out a raid in Lucknow, which brought the entire illegal operation into the open.

Is the cough syrup illegal or adulterated?

No. The medicine itself is neither fake nor adulterated. Codeine is an opioid that is legally manufactured and sold, but only in limited quantities and strictly through authorised sellers with a valid prescription.

What the Uttar Pradesh government is acting against is the illegal stocking and smuggling of codeine-based cough syrups. These syrups were diverted from the legal supply chain and sold for non-medical, addiction-inducing use.

On 8 December, FSDA (Food Safety and Drug Administration) Commissioner Roshan Jacob clearly addressed this issue during a joint press conference in Uttar Pradesh. She stated that all the cases identified so far relate to illegal stocking, illegal sale, and smuggling of cough syrups, and not to adulteration.

The seized syrups fall under Schedule H drugs. These are legal medicines meant for human use, but they can only be sold with a medical prescription. The crime lies in breaking regulatory rules, selling without prescriptions, and trafficking the drug illegally.

Given the large scale of the operation and the clear intent behind it, the case has been booked under the Narcotic Drugs and Psychotropic Substances (NDPS) Act. This is the reason why the accused are finding it difficult to secure bail.

Is there any connection to the Tamil Nadu cough syrup deaths?

Absolutely not.

The tragic deaths of children in Madhya Pradesh and Rajasthan that were linked to cough syrup manufactured in Tamil Nadu are a completely separate matter. That case is currently being investigated by the central government. It involved adulterated cough syrups that were found to contain DEG, a highly toxic substance.

There has been no instance of child deaths linked to cough syrup in Uttar Pradesh. Any attempt to link these two cases is a deliberate effort to mislead the public.

The Uttar Pradesh case deals strictly with the illegal trafficking of otherwise legal medicines. It has nothing to do with deaths caused by contaminated or adulterated drugs.

Is codeine cough syrup used for intoxication?

Yes, when it is misused.

From a medical point of view, this syrup is prescribed to treat cough and pain. It contains codeine, an opioid derived from opium. Normally, these syrups contain about 10 to 20 mg of codeine per 5 ml.

When taken exactly as prescribed by a doctor, it works like any other legitimate medicine. However, investigations have shown that drug users consume the syrup in very large quantities. This excessive intake leads to intoxication, with effects that can last for six to eight hours because of the high dose of codeine.

Why does a ₹120 syrup sell for ₹1,500 illegally?

This is where the economics of misuse and smuggling become clear.

In Muslim-majority countries like Bangladesh, and in Indian regions with a high Muslim population such as parts of Uttar Pradesh, Jharkhand, Bihar, West Bengal, Assam, Tripura, and Sikkim, this cough syrup is being misused as an alternative intoxicant.

Since alcohol is considered haram in Islam, codeine-based cough syrup has come to be seen by some users as a so-called “halal” substitute for intoxication. Because of this demand, a bottle that legally costs between ₹120 and ₹160 is sold in the black market for anywhere between ₹1,200 and ₹1,500.

The bottom line

The Uttar Pradesh cough syrup case is solely about the illegal storage, sale, and trafficking of prescription medicines. There is no evidence of fake drugs, adulterated syrups, or deaths linked to these cough syrups in Uttar Pradesh.

Efforts to connect this case with the Tamil Nadu cough syrup deaths are aimed only at creating confusion and undermining the Yogi Adityanath government’s firm action against a deeply rooted drug trafficking network.

The facts are straightforward. The misinformation is intentional.
